Allen Health Clinic would like to help make your office visit run as smoothly as possible. To help facilitate this process we have provided our patient forms online for you to download and complete before your scheduled appointment.
For your convenience our forms are fill-able forms. Once you complete and print the forms, please sign where indicated.
Should you need assistance in obtaining paper copies of any of our forms, please call our office and we will mail you a packet.
If you are a current patient please update forms every 2 years or if there has been a change to your contact information, insurance, or significant changes in your health history.
* If a minor, Legal Guardian Consent form must be completed in person by legal guardian and can not be faxed.
